Update model card with Primus paper and segmentation tag

This PR improves the model card for the Primus-M checkpoint by:

Updating the pipeline_tag to image-segmentation to accurately reflect its application.
Adding library_name: nnunet as the model is designed for use within the nnU-Net framework.
Linking the paper Primus: Enforcing Attention Usage for 3D Medical Image Segmentation which introduces the architecture.
Including relevant citations and usage instructions from the official documentation.
